Keratoacanthoma has been described as a benign lesion that spontaneously involutes and as a variant of squamous cell carcinoma that rarely can behave aggressively. We present a case of a keratoacanthoma-like lesion in an elderly patient that recurs invading deeply into vital structures.
The diagnosis of a classic keratoacanthoma (KA) is based on clinical and histologic findings. Clinically, the classic KA appears as a nodule with a central keratin-filled crater. It has three phases of behavior:
(1) a proliferative phase of 2?10 weeks,
(2) a stationary period of 2-10 weeks, and
(3) an involution phase of up to one year duration.
KA has distinct histological features of keratin-filled crater lined by a proliferating squamous epithelium. Shave or incisional punch biopsies are usually inadequate to diagnose keratoacanthoma, and the definite diagnosis can only be established after tumor excision, based on the histological findings.
Treatment of classic keratoacanthomas can be conservative given their self-limiting course and has included curettage and intralesional methotrexate.
The catch with KA is the association with squamous cell carcinoma (SCC) and occasional aggressive behavior. 16.7% of KA are SCC. This speaks to the significant overlap in the clinical and histological appearance of the two, which can result in misdiagnosis of SCC as KA. In addition, KA may coexist with a SCC in the same lesion, as the more aggressive cell line eventually transforms the tumor into an aggressive lesion. The development of SCC in a KA has been reported in 5.7% of KA?s. In patients older than 90 years, the transformation incidence was found to be almost 14%.
Metastases with KA?s are very rare even with evidence of perineural invasion. It has been suggested that perineural invasion in KA?s is less significant than in SCC.
Treatment of keratoacanthomas is surgery. Mohs surgery is the standard with a reported recurrence rate of 2.4%. Radiation including orthovoltage has been shown to be highly effective as well.
In our case report, an 86 y.o. woman underwent excision of a right lateral canthus keratoacanthoma with Mohs surgery. The lesion recurred 2 months later spreading into the orbit and deep along the surface of the maxilla. She required a partial maxillectomy and orbital exenteration with post-operative radiation therapy. Forehead fasciocutaneous flap was used for reconstruction.

John M. Robinson, the Chief Diversity Officer at the U.S. Department of State, wants America?s diplomats to know that common phrases and idioms like ?holding down the fort? are, in fact, deeply racist.
Robinson, who also serves as director of the Department?s Office of Civil Rights, used his ?Diversity Notes? feature in the July/August issue of the official ?State Magazine? to examine the hateful roots of everyday sayings. In one recent public relations kerfuffle at Nike, Inc., he wrote, the company torpedoed a sneaker called the ?Black and Tan.?
?What a wonderful celebratory gesture and appreciation for Irish culture. Not!? wrote Robinson, an adult.
Robinson notes that ?Black and Tan,? in addition to being an enjoyably robust alcoholic concoction,?can refer to the brutal Protestant militiamen who ravaged the Irish countryside in the early 20th century ? which is why Irish bartenders always get so upset when you order one.
In an effort to avoid offending those notoriously fragile Irish sensibilities, Nike pulled the shoe from stores.
Robinson would like us all to learn from the sneaker company?s inadvertent racism and really start watching what we say. For example, did you know ?going Dutch? is a reference to Netherlanders? apparently well-known parsimoniousness, and that your widowed neighbor, sweet old Mrs. Rasmussen, cries every time she hears you use it?
And did you know using the phrase ?holding down the fort? is the linguistic equivalent of scalping a Cherokee? According to Robinson, the phrase dates back to American soldiers on the western frontier who wanted to ?hold down? all that land they stole.
?Handicap? and ?rule of thumb? are two more figures of speech that Robsinon, in his wisdom, has decreed offensive. The latter, Robinson says, refers to the width of a stick a man could once use to legally beat his wife.
And in case you?re wondering how he could have done all the etymological detective work necessary to conclude that these phrases came from where he says they came from, and still have time to perform his Chief Diversity Officer duties at the State Department, wonder no more: Robinson doesn?t really know if any of this is true.
?Much has been written about whether the etymologies below are true or merely folklore, but this isn?t about their historical validity,? Robinson writes. ?[I]nstead, it is an opportunity to remember that our choice of wording affects our professional environment.?

(Reuters) - A 15-year-old boy suspected of shooting a fellow student on the first day of classes at a Baltimore-area high school has been charged with first-degree attempted murder, but police say they do not know the motive for what apparently was a well-planned attack.
Robert Wayne Gladden Jr. also left an ominous message on his Facebook page before heading off to Perry Hall High School in Baltimore's northeast suburbs on Monday morning.
"First day of school, last day of my life," the posting read. It concluded: "fuck the world."
Gladden was charged Tuesday as an adult and also faces a first-degree assault charge, Baltimore County Police officials said. He is being held without bail ahead of a preliminary hearing set for September 7.
The shooting is the latest incident of gun violence in a summer plagued by troubling shootings in the United States.
In July, a gunman killed 12 people and wounded 58 at a movie theater in Aurora, Colorado. In August, a suspect described by authorities as a white supremacist killed six people at a Sikh temple in Wisconsin before committing suicide.
Gladden has been cooperating with the investigation, but police said they still do not know what led him allegedly to open fire in the school cafeteria around 11 a.m..
Counter to press reports, Baltimore County Police Corporal Cathy Batton said Gladden was not a victim of bullying. "He denied being bullied," Batton said. "That was not the reason."
"When he walked into the cafeteria and pulled the trigger he did not have any intended target," Batton said.
Batton said Gladden had carefully planned the attack.
Gladden disassembled a double-barreled shotgun he took from his father's house, packed it into his school bag along with a bottle of vodka and hid the weapon in a school bathroom, Batton said. The teen allegedly returned, re-assembled the weapon, drank some of the vodka, and went back into the cafeteria.
A guidance counselor rushed and tackled Gladden when he saw the student produce the gun, but Gladden fired one shot into a crowd of students, striking classmate Daniel Borowy in the back, Batton said. Borowy remains hospitalized in critical condition.
In the work and education section of the boy's Facebook page, located under the handle "Bobby Gladden (SuicidalSmile)," he lists attending Perry Hall but also working for "The Manson Family," an apparent reference to Charles Manson, who is serving a life sentence for directing the 1969 murder of actress Sharon Tate and six others.
The "About Bobby" section reads: "im a meathead, and that's about it."

WASHINGTON (AP) ? As tropical storm Isaac bears down on the Gulf Coast, there should be plenty of money ? some $1.5 billion ? in federal disaster aid coffers, thanks, in part, to a new system that budgets help for victims of hurricanes, tornadoes and floods before they occur.
It's a system that Paul Ryan, the Republican nominee-to-be for vice president, had hoped to scrap as a way to make his House GOP budget look smaller by about $10 billion a year. Politely, party elders told him no way, at least for now.
The Obama administration was the driving force behind the new disaster funding scheme and made it part of last summer's hard-fought budget pact, even though President Barack Obama himself had given short shrift to budgeting for disasters prior to a spate of them early last year, including tornadoes that ripped through Missouri and Alabama.
Congresses and administrations, after all, always had been fairly forthcoming with whatever disaster aid was needed after the fact.
The new system means disaster aid will not have to compete with other programs for financing, nor have to rely on less certain ad hoc funding at the height of a crisis.
Instead, disaster money was added on top of the official budget "cap" in line with the amounts budgeted in prior years.
It had been a different story earlier in the year as the government's chief disaster fund almost ran dry, thanks to foot-dragging by the White House and demands by tea party House Republicans that disaster aid be partly "paid for" with cuts to programs that Obama favored. The administration instead let the political pressure build as disaster accounts dwindled, sparking the ire of both his GOP rivals and allies like Sen. Mary Landrieu, D-La., but turning the tables in its favor.
Months after agreeing to the new regime, Republican leaders still had to turn to procedural maneuvers to orchestrate passage of $8.8 billion in disaster money in keeping with the agreement. Ryan, the House Budget Committee's chairman, was among 66 Republicans opposing the measure.
But, as has been proven time after time ? especially as tornadoes and hurricanes rip through politically conservative states ? even the sturdiest tea party supporters like the government when it comes to doling out money to storm victims for motels and other temporary housing and to help with house repairs.
Simply put, when disaster strikes, politicians respond ? sometimes to the dismay of conservatives who see politicians milking disasters to win projects that couldn't fly on their own, like Mississippi Republican Sen. Thad Cochran's failed bid six years ago for $700 million to relocate a rail line along the Gulf Coast so his state could build a new east-west highway.
The new system makes it more difficult for lawmakers to try such maneuvers, though such "pork barrel" tactics largely have been halted anyway with the GOP takeover of the House, with Republicans instituting a ban on so-called earmarks.
Many House Republicans, however, are unhappy with the new approach. Ryan earlier this year tried to gut it and eliminate $10 billion a year in disaster costs when putting together the House GOP budget.
But in doing so Ryan sideswiped a still-powerful Appropriations Committee that was still stinging from $19 billion in Budget Committee-induced cuts to last year's deal. Appropriations Committee Chairman Harold Rogers, R-Ky., protested to GOP leaders and won a gentleman's agreement that the new system would stay in place, at least for this year.
However, Ryan's point of view might still prevail if Mitt Romney defeats Obama in November.
What Ryan proposes is that when disaster strikes, lawmakers scour the rest of the budget for savings to pay for rebuilding homes, roads and schools and helping small businesses. Put another way, instead of being an excuse to increase spending, disasters would offer an opportunity to make further cuts elsewhere in the government.
That's easier said than done. The portion of the budget that would face cuts ? the day-to-day operating expenses for federal agencies ? already has absorbed several rounds of cuts under GOP control of the House and faces almost a decade's worth of tight limits under last year's budget deal.
And should a huge disaster like Hurricane Katrina hit, it's simply unrealistic to find cuts of the magnitude required. Katrina ended up costing taxpayers more than $60 billion. Much of that money was approved when images of hurricane victims were still fresh in the minds of lawmakers and voters and as the George W. Bush administration continued to reel from its initial botched response.
It's also easier to summon the political fortitude to demand that disaster relief be paid for when the area you represent hasn't been hit. In particularly bad disaster years, political pressure accumulates as the toll of Midwestern floods, Southern tornadoes and coastal storms is felt by more and more voters and their representatives.
That's exactly what happened last year in the wake of Hurricane Irene, which prompted liberals like Rep. Peter Welch, D-Vt., and tea party-backed Republicans like Rep. Nan Hayworth of New York to form a coalition to press for aid.

The bad economy and their unhappiness with Obama have voters ready to leave him, but they've got to feel a little better about Romney to make the leap.?
